Advance Care Directive

An advance care directive is a legal document where you write down your wishes for medical treatment in situations where you cannot speak for yourself — for example, if you are unconscious or have a serious illness. It tells doctors and your family what treatments you want and do not want. Different countries and states use different names for this document, but the purpose is the same: making sure your medical wishes are followed even when you cannot express them yourself.
